The Your Community PK website has been developed as an initiative of the Perth and Kinross Health and Social Care Partnership.

The Perth and Kinross Health and Social Care Partnership was established to bring together adult health and social care services. The new Perth and Kinross Health and Social Care Partnership and its Integration Joint Board (IJB) will deliver integrated health and social care services for adults across Perth and Kinross.

Why integrate health and social care services?

We need to change the way we deliver care to meet the needs of our growing elderly population and people living with long-term conditions. It’s good news that people are living longer and the vast majority of older people live healthily at home. We know, however, that the need for support from care services  increases with age. The challenge for services and communities will be to ensure that people are able to lead healthy, fulfilling lives at home for as long as possible.

You have told us that current systems are too complicated: you want to see them joined up, with clear information about who to contact. People also want to see services delivered closer to their homes, and for people using services, their families and carers to be involved in getting the right care.

To help these changes to happen your local care services are now provided through the Perth and Kinross Health and Social Care Partnership, made up of representatives from Perth & Kinross Council, NHS Tayside and other community partners.

Our Health and Social Care Partnership is led by the Integrated Joint Board, who will work with a single vision and joint budget that will deliver services with communities, in new ways and support people to lead active, independent and healthy lives in their own communities.

The Health and Social Care Partnership are working in line with the 2017-2027 Community Plan / Local Outcomes Improvement Plan for Perth & Kinross. More information about the objectives of the Community plan can be found on the Perth and Kinross Community Planning Partnership website.
